Sacramento, CA — February 1st, 2019 — Starting February 1st, Path to Health enrollment is available through two providers in Sonoma County:
About Path To Health
Path to Health is a pilot project of the CMSP Governing Board that launched on February 1, 2019 and extends through January 31, 2022. Path to Health is currently enrolling up to 25,000 undocumented residents within 35 designated California counties who are:
- Ages 26 and older
- Already enrolled in restricted scope/emergency only Medi-Cal but lacking coverage for preventative health services and routine prescription medications
- Living in a participating county
