Become a Path to Health Provider
Sacramento, CA — June 5, 2020
Path to Health is now seeking additional Federally Qualified Health Centers, Rural Health Clinics or Tribal Health Program providers to join the Path to Health Pilot Project. Please review the Invitation to Apply and complete a Clinic Enrollment Form by no later than July 6, 2020. If you miss the deadline, the next chance to enroll as a Path to Health provider will be September 2020.
ABOUT PATH TO HEALTH
Path to Health is a pilot project of the CMSP Governing Board that launched on February 1, 2019 and extends through January 31, 2022. Path to Health is currently enrolling up to 25,000 undocumented residents within 35 designated California counties who are:
- Ages 26 and older
- Already enrolled in restricted scope/emergency only Medi-Cal but lacking coverage for preventative health services and routine prescription medications
- Living in a participating county
